Приверженность лечению беременных с различными видами патологии: клинико психологический обзор зарубежных исследований

Аннотация

Введение. В период беременности приверженность лечению приобретает особое клиническое и психологическое значение, поскольку выполнение медицинских рекомендаций связано не только с состоянием женщины, но и с благополучием плода. При этом приверженность беременных существенно различается в зависимости от вида патологии, характера заболевания, формы лечения, воспринимаемой безопасности терапии и качества коммуникации с медицинскими специалистами. Цель обзора — обобщить данные зарубежных исследований о приверженности лечению беременных с различными видами соматической, инфекционной и психической патологии и выделить нозологически специфичные и сквозные клинико-психологические барьеры соблюдения медицинских рекомендаций. Материалы и методы. Выполнен тематический анализ материалов, выделенных из более широкого обзора о приверженности лечению в период беременности. В фокус анализа включены публикации, посвященные антиретровирусной терапии при ВИЧ-инфекции, лечению сахарного диабета и гестационного сахарного диабета, профилактике и терапии железодефицитной анемии, антигипертензивной терапии и самостоятельному мониторингу артериального давления, противоэпилептической терапии, а также психотерапии и психофармакотерапии в период беременности. Результаты. Наиболее выраженные барьеры приверженности беременных женщин связаны с дефицитом знаний о заболевании и лечении, страхом вреда для плода, побочными эффектами терапии, финансовыми и организационными ограничениями, стигматизацией, депрессивной и тревожной симптоматикой, низкой самоэффективностью, недостаточной социальной поддержкой и низким доверием врачу. При этом мотивация, ориентированная на благополучие будущего ребенка, понятное объяснение пользы и безопасности лечения, опыт предшествующей терапии и поддержка ближайшего окружения могут повышать приверженность. Выводы. Приверженность лечению беременных нельзя рассматривать как единый универсальный показатель. Ее содержание и барьеры существенно меняются в зависимости от клинического контекста: при ВИЧ-инфекции доминирующими барьерами являются стигма и доступность помощи, при гестационном сахарном диабете — принятие диагноза и изменение образа жизни, при анемии — доступность препаратов и переносимость терапии, при гипертензивных состояниях — регулярность самоконтроля,
при эпилепсии — баланс рисков приступов и тератогенных опасений, при психических расстройствах — амбивалентность и страх вреда для ребенка. Перспективное направление — разработка дифференцированных медико-психологических программ повышения приверженности лечению в период беременности, учитывающих состояние здоровья пациентки, характер терапии и ведущие барьеры соблюдения медицинских рекомендаций.
